What we'll ask for

Certification number
From ASCP (PBT), NHA (CPT) or AMT (RPT). We confirm it with the issuer.
State license
Your license number if you work in a state that licenses phlebotomists.
Certificate of insurance
Professional liability coverage, as a PDF from your insurer.
Photo ID
So we can match you to your certification. It's never shown on your listing.

You won't be charged until your checks pass. If they don't, you pay nothing.
Questions phlebotomists ask
Do you take a cut of my visits?
No. Patients pay you directly. The yearly listing fee is the only charge.
I work for an agency. Can I still list?
Yes, if you also take your own clients. Each phlebotomist applies and is verified separately, and can list under the business name.
What if my certification lapses?
Your listing is hidden until it's current again. Your remaining months carry over.
Can I cancel?
Cancel anytime from your account. Your listing stays up until the end of the year you've paid for, then it doesn't renew. Once your listing is approved, the yearly fee isn't refundable.
